Our office is required by the federal government to provide all patients with a written copy of our HIPPA Policy and to obtain written acknowledgement of this from the patient. It states that the patient acknowledges the receipt of our office “HIPPA Notice Form” which can also be found listed under the “Forms” tab on this website.
